Our office hours are 8:00 to 5:00 Monday through Friday

If you have questions about your medical record, you may call (360) 397-3638 during our office hours and our Release of Information (R.O.I.) Specialist will assist you.

If you would like to review your medical record, you may do so by scheduling an apppointment with our Health Information Manager. All reviews are done at our Columbia Tech Center office; the phone numbers to schedule an appointment are (360) 397-3611. Please remember to bring picture ID with you to the appointment.
